helps find assisted living or any type of senior housing or elder care solutions. They support the transition from one home to another, or help find the services one might need to remain in their home.
With financial assistance from 华体会, Elderwerks offered the Elderwerks Aging Better Senior Expo, a virtual event offered at no cost for seniors and their families to learn how to plan for their futures regarding health, finance, care, benefits and living.
Erie Family Health Center (Erie) was founded in 1957 as a project of volunteer physicians from Northwestern Memorial and Erie Neighborhood House. Erie provides a variety of primary care, case management and dental services to over 37,500 low-income, underinsured and uninsured patients per year.
Erie鈥檚 eleven community-based health centers are portals to culturally sensitive, high quality healthcare鈥攔egardless of a patient鈥檚 ability to pay.
Based in Crystal Lake, Illinois, works to better the health of McHenry County communities. With compassion and respect, the organization improves access to care by providing a broad spectrum of services to residents who experience barriers to health and wellness.

With support from 华体会 and others, the provides leadership to ensure the prevention and treatment of mental illness, developmental disabilities and substance abuse by identifying, planning, coordinating, fostering development, and contracting for quality services for all citizens of McHenry County.
Financial support from 华体会 has been used to offer a full-day instructional conference at no cost for professionals and the community to discuss concerns, successes and challenges, and share tools and best practices with all who attend.
Near North Health Service Corporation (Near North), our community health partner for more than 40 years, provides comprehensive primary care, social services and dental services to primarily low-income uninsured or underinsured residents who live in some of the city鈥檚 most medically underserved communities.
To address increasing community needs, Near North expanded to provide clinical and social services through eight health centers, making it one of the largest providers of community-based primary care in Chicago. In fiscal year 2013, Near North served 46,130 patients through more than 102,018 visits.
